Active chest decompression is achieved when the rescuer maintains a firm grip on the RESQPUMP, leans at the waist, and lifts their own weight upward after each compression. The suction cup adheres to the chest and transfers the lifting force to the mid-chest. The compression force is transferred to the chest in the same way as in standard CPR, through the device's piston.

The handle of the RESQPUMP integrates a battery-powered metronome, which guides the rescuer on the appropriate rate of compression and decompression. The metronome emits signals in two tones of equal duration, a low tone and a high tone, to guide the rescuer in compression and decompression at the appropriate rate and for an equal duration (50% duty cycle).
